Prescription Fraud Reporting Program
Welcome to Texas Pharmacy Association's Prescription Fraud Reporting Program. You can use the form below to report fraudulent prescription activity or stolen prescription pads to TPA. After we receive the report, an alert will be sent by email to pharmacists and pharmacy technicians in your area. Please complete as much of the information as possible to allow for reasonable identification, but do NOT include suspect names to reduce possible liability issues. To Report Prescrption Fraud or Prescription Pad Theft:
- Fill out the online form, completing all information requested.
- When finished, use the link at the bottom of the page to submit the form by email; or
- Print the form out and fax it to 512-836-0308. Be sure your cover sheet says "Prescription Fraud Alert" in the fax subject line.
Do not use this form to report pharmacy thefts or robberies. Report all pharmacy robberies or product thefts to local law enforcement.
